Lake Plains Eye Center Overview
Staff
and Doctors welcome you. We would be happy to make an appointment for you
with the Premier Eye Facility in Orleans County. Our practice specializes in diagnosis,
treatment, and consultation of Disease which effects the eyes. We Provide Adult and
Pediatric Ophthalmic Services. Home and nursing visits are available, please call to

Your Office Visit
- First Visit: At the time of your first visit, you will be asked to fill out a medical information form. It is important that you bring your glasses or contacts, a list of your medications with dosages, a list of drug allergies, insurance cards. co-pay and any other information that you feel would be helpful. If you need specific information,
